Clinical privilege requests
How do we take a practitioner's request for new clinical privileges, collect the training and case logs, and route it to the department chair and the committee for a recorded decision?
Run the privilege request as a criteria-based file with the decisions in sequence. When a practitioner requests new or expanded privileges, Callvu classifies the privilege against the department's delineation and its criteria (the training or fellowship, the case numbers and outcomes, the proctoring, the certification), chases the evidence from the practitioner, the training program and the prior institutions through their links, verifies what the standards require from the primary source, routes the file to the department chair and then the credentials committee and the board in the Decision Desk with only the allowed outcomes, and writes the granted privileges with any proctoring conditions and the review date to the credentialing system and the EHR's order and scheduling rules.

The problem today
The surgeon had been scheduling the new procedure for two months on the strength of a verbal from the chair; the committee had not met.
Privileges are what a hospital permits a practitioner to do, and granting them is a medical staff decision with criteria, evidence and a sequence of approvals. In practice the request is a form, the case log is an email, the chair's approval is a signature on paper, and the committee's decision waits for the quarterly meeting while the scheduling system has no idea any of it is pending.
The accreditation standards require privileges to be granted on criteria, with the evidence verified at the primary source, decided by the organized medical staff and approved by the governing body, with focused evaluation on new privileges. The scheduling and ordering systems should allow only what was granted. The file is a set of documents from named parties against named criteria.
A privilege request that arrives complete against the criteria is decided at the next meeting, and the scheduling system knows the morning after.
How it runs
From the requestto the system of record.
A practitioner requests new or expanded privileges through the medical staff office. Callvu opens the request against the department's delineation.
- Practitioner
- Requests the privilege, provides training evidence and case logs, and attests through a link.
- Training programs and prior institutions
- Verify training, case volumes and standing through their links.
- Department chair, credentials committee and board
- Decide in sequence in the Decision Desk.
- Medical staff office
- Manages the file and the proctoring plan.
- Callvu
- Classifies, chases, verifies, routes in sequence and writes.
Classify the privilege
The requested privilege is matched to the department's delineation of privileges and its criteria: the required training or fellowship, the minimum case numbers over the look-back with outcomes, the certification, the proctoring or focused professional practice evaluation on grant, and any equipment or team requirement; the practitioner's current privileges and credentials are prefilled from the credentialing system.
Chase and verify
The evidence is requested from the parties that hold it through links (the practitioner for the case logs and the attestation, the training program for the completion and the procedure volumes, prior institutions for the privileges held and the quality data) and chased on the committee's calendar; each is verified at the primary source where the standards require and checked against the criteria, with gaps returned with the specifics.
Decide in sequence
The complete file goes to the department chair in the Decision Desk with the criteria and the evidence, then to the credentials committee and the medical executive committee, then to the board, each with only the allowed outcomes: recommend or grant, grant with proctoring or conditions, defer with specifics, or deny with the reasons and the practitioner's hearing rights under the bylaws.
Write back
The granted privileges with the conditions, the proctoring plan and the review date are written to the credentialing system, the EHR's order and scheduling rules are updated so the practitioner can schedule the procedure, the proctoring cases are tracked until the focused evaluation closes, and the privilege joins the reappointment cycle.
Where it ends: The privilege was granted on criteria with verified evidence, by the chair, the committee and the board in sequence, and the credentialing system and the EHR allow exactly what was granted with the proctoring tracked. The Decision Trace shows the criteria, each verification and each decision; the Audit Trace shows the requests, the verifications and the writes.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The practitioner through the medical staff office; programs and institutions through their links.
The back office
The chair, the committee and the board decide complete files; the medical staff office tracks proctoring from the record.
The Decision Desk
- The complete file goes to the department chair, then the credentials committee and the board in the Decision Desk, each with the criteria and the evidence and only recommend or grant, grant with proctoring, defer with specifics, or deny with reasons and hearing rights.

Questions
What buyers ask next,answered plainly.
What evidence is required?
What the delineation's criteria name for the privilege: the training or fellowship, the case numbers and outcomes over the look-back, the certification, verified at the primary source, plus the practitioner's attestation.
Who approves?
The department chair, the credentials committee and the medical executive committee, then the board, in the bylaws' sequence, each in the Decision Desk with the file.
When can the practitioner schedule the procedure?
When the board's grant is written to the credentialing system and the EHR's scheduling and order rules are updated, with the proctoring cases tracked.
