Provider credentialing
How do we credential a practitioner with the licenses, board certification, malpractice history and work history collected, primary-source verified, and sent to the credentials committee inside the standards' timelines?
Run credentialing as a checklist the practitioner fills once and the process verifies at the source. When a practitioner joins, Callvu prefills the application from the practitioner's existing profile (the national credentialing database where the organization uses it, the national provider identifier, the licensing boards), lists the elements the standards require for the practitioner type, chases the attestation, the documents and the explanations through the practitioner's link, runs the primary-source verifications (the license, the certification, the education and training, the federal exclusion and sanction lists, the practitioner data bank query, the malpractice carrier's history, the work history gaps), routes the complete file to the credentials committee in the Decision Desk with every verification's result and date, and writes the decision with the effective date to the credentialing system of record, the provider directory and the billing system.

The problem today
The surgeon started on the first; the credentialing file was approved in the fourth month; nothing in between could be billed.
Credentialing is a known list of elements with known sources, and it still takes months: the application arrives incomplete, the work history has a gap nobody asked about, the malpractice history takes a letter to the carrier, the primary-source verifications are done one by one by hand, and the committee meets monthly. The practitioner cannot see patients under the plan or bill, and the department covers the schedule with locums.
The NCQA credentialing standards and the Joint Commission medical staff standards set the elements (current license, drug enforcement registration, education and training, board certification, work history for five years with gaps explained, malpractice history, sanctions and exclusions, the practitioner data bank query, the attestation), the sources that count as primary, the age a verification may have at the committee's decision (one hundred eighty days for most), and the committee's role. Each element is a chase to a source with a date, which is what a process does.
A file where every verification has a source and a date, inside the age limits, is a file the committee decides on the day.
How it runs
From the requestto the system of record.
A practitioner is hired, contracted or applies to join the network or the medical staff. Callvu opens the credentialing file with the practitioner type and the deadline.
- Practitioner
- Completes and attests to the application, uploads the documents and explains any gap or history through their link.
- Credentialing team
- Runs the verifications the process cannot complete automatically, resolves discrepancies and assembles the file.
- Credentials committee
- Decides the application in the Decision Desk with the verified file.
- Callvu
- Prefills, lists the elements, chases, runs and dates the verifications, routes, writes and tracks the age limits.
Prefill and list the elements
The application is prefilled from the national credentialing database profile where the practitioner maintains one, the national provider identifier registry and the state licensing board's public record; the practitioner type (physician, advanced practice, allied health) and the setting (health plan network, hospital medical staff) classify the elements the standards require and the verification sources for each.
Chase and verify from the practitioner
The practitioner completes what is missing, attests to the questions the standards require (the ability to perform, the absence of illegal drug use, the license and privilege history, the malpractice and sanction history, the correctness and completeness of the application), uploads the documents (the license, the drug enforcement registration, the certification, the malpractice coverage face sheet, the education and training certificates) and explains any work history gap longer than the standard allows or any disclosed history, through their link, chased on the clock.
Run the primary-source verifications
Each element is verified at its primary source with the date and the result recorded: the license with the state board, the drug enforcement registration, the board certification with the certifying board, the education and training with the institutions or the recognized equivalent source, the practitioner data bank query, the federal exclusion and sanction lists (the exclusion database, the system for award management), the state sanction history, the malpractice history with the carrier, and the work history against the application; a discrepancy (a lapsed license, an unreported action, an unexplained gap, a data bank report) is routed to the credentialing team with the specifics for resolution with the practitioner, and the file tracks each verification's age against the standards' limits.
Decide and write back
The complete, verified file goes to the credentials committee (or the medical director for a clean file where the standards allow) in the Decision Desk with every verification's source, date and result and only the allowed outcomes: approve, approve with conditions, defer for named information, or deny with the reasons and the appeal rights; the decision with its effective date, the next recredentialing date and the verified elements is written to the credentialing system of record, and from it to the provider directory, the privileging record and the billing system so the practitioner can be scheduled and billed.
Where it ends: The practitioner was credentialed on a file verified at the source, inside the standards' age limits, decided by the committee with the results in view, and the credentialing system of record, the directory and the billing system reflect the effective date. The Decision Trace shows the elements required, each verification's source and result, and the committee's decision; the Audit Trace shows the application, the chases, each verification's date and the write.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The practitioner through their link, prefilled from their existing profile; the credentialing team sees the status.
The back office
The credentialing team resolves discrepancies rather than keying applications; the committee decides verified files.
The Decision Desk
- The complete file goes to the credentials committee in the Decision Desk with every verification's result and only approve, approve with conditions, defer for named information, or deny with reasons and appeal rights; a discrepancy goes to the credentialing team for resolution before the committee sees it.

Questions
What buyers ask next,answered plainly.
What is verified at the primary source?
The license, the drug enforcement registration, the board certification, the education and training, the practitioner data bank, the exclusion and sanction lists, the malpractice history and the work history, each with the source, the date and the result on the file.
How old may a verification be?
Most no older than one hundred eighty days at the committee's decision under the NCQA standards, and the decision within one hundred eighty days of the practitioner's attestation; the file tracks the age of each.
What happens after approval?
The decision with its effective date is written to the credentialing system of record and from it to the provider directory, the privileging record and the billing system, with the recredentialing date set.
