Force-placed insurance notices and cancellation
How do we send the force-placed insurance notices on the Regulation X schedule, and cancel the policy with a refund within 15 days when the borrower proves coverage?
Run the sequence on the rule's clock and make the proof a link. Callvu starts the sequence from the lapse event, sends the first notice with the rule's content at least 45 days before any charge and the reminder at least 15 days before, gives the borrower a link to upload evidence of coverage at every step, reads and checks the evidence when it arrives, classifies the outcome (coverage proven for the whole period, for part of it, or not), routes exceptions to the insurance analyst in the Decision Desk, binds the force-placed policy only when the clock has run without evidence, and on later evidence cancels the policy and refunds the overlapping premiums within 15 days, writing every notice, charge and refund to servicing.

The problem today
The borrower sent proof of coverage twice, and the force-placed premium stayed on the account for five months.
The servicer force-places a policy after a lapse. The borrower, who had coverage all along through a new carrier, faxes the declarations page to a number on the notice. The fax is filed. The premium is added to the escrow, the payment rises, the borrower complains, and the refund comes after the regulator's letter.
Regulation X is specific: two notices with prescribed content and timing before any charge, a reasonable basis before the charge, and on evidence of coverage, cancellation within 15 days with a refund of premiums for any period of overlapping coverage. Charges must be for a bona fide and reasonable cost. Servicers have paid large sums in enforcement and settlements over this sequence, mostly for charging without the notices, overlapping coverage and slow refunds.
The rule is a clock with a proof window; the process should be the same.
How it runs
From the requestto the system of record.
A hazard or flood insurance lapse is established on a loan (through the renewal process) with a reasonable basis. Callvu starts the force-placed sequence on the loan.
- Borrower
- Receives the notices and uploads evidence of coverage through a link at any point.
- Insurance analyst
- Decides exceptions in the Decision Desk.
- Force-placed carrier
- Binds and cancels the policy on instruction.
- Servicing
- Holds the notices, the charge and the refund.
- Callvu
- Runs the clock, sends the notices, reads evidence, classifies, routes, instructs the carrier and writes.
Send the first notice
The first notice is generated with the rule's content (the lapse, the request for evidence, how to provide it, that the servicer will purchase coverage at the borrower's cost and that it may be more expensive and cover less, the contact) and sent at least 45 days before any charge, with a link to upload evidence; the clock starts.
Chase and verify evidence
At any point the borrower uploads evidence through the link or the agent sends it; it is read and checked (the property, the coverage, the mortgagee clause, the dates) and the outcome classified: coverage proven for the whole period (stop the sequence), for part (adjust the charge period), insufficient (continue with the specifics explained). The reminder notice goes out at least 30 days after the first and at least 15 days before any charge where no sufficient evidence has arrived.
Decide and bind
When the clock has run with no sufficient evidence, the force-placed policy is bound by rule for the lapse period with the bona fide and reasonable charge and the notice of the charge; an exception (partial evidence, a dispute, an escrowed loan where the premium should have been paid) goes to the insurance analyst in the Decision Desk with only the allowed outcomes.
Cancel, refund and write back
On later evidence of coverage, the force-placed policy is cancelled within 15 days, the premiums for the overlapping period are refunded, and the escrow is corrected; every notice with its date, the charge, the cancellation and the refund are written to the servicing system.
Where it ends: No charge was made without the two notices on the clock and a reasonable basis, every piece of evidence was read within the window, and any overlap was refunded within 15 days. The Decision Trace shows the clock, the evidence checks and the decisions; the Audit Trace shows each notice, upload, charge and refund with its time. The Callvu moves in this process are classify, chase and verify, decide and write back.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The borrower meets the process in the notices and the proof link, in any channel.
The back office
The insurance analyst sees exceptions; the carrier is instructed from the record; servicing holds the dates.
The Decision Desk
- Partial evidence, a dispute, or an escrowed loan in the sequence goes to the insurance analyst in the Decision Desk with the checks and only stop, adjust the charge period, or continue with specifics.

Questions
What buyers ask next,answered plainly.
What notices are required before a force-placed charge?
A first notice at least 45 days before the charge and a reminder at least 30 days after it and at least 15 days before the charge, each with the prescribed content; the sequence will not bind without both.
What happens when the borrower proves coverage?
The force-placed policy is cancelled within 15 days and the premiums for the overlapping period refunded, with the escrow corrected and the record written.
Does this apply to escrowed loans?
An escrowed loan's premium is paid rather than force-placed unless the policy was cancelled for a reason other than non-payment; the sequence checks before it starts.
