Provider claim disputes

How do we take a provider's dispute of a claim payment, gather the claim, the contract terms and the records, and decide it on the contract's and the state's clock?

Treat the dispute as a case with the contract inside it. When a provider disputes a payment, Callvu captures the dispute through the provider's link with the claims and the reason, classifies it (an underpayment against the contract rate, a denial for medical necessity or authorization, a timely filing or coding denial, a bundling or modifier edit, a coordination of benefits question), confirms the dispute is inside the contract's and the state's dispute window, prefills the claim's adjudication, the contract terms and the fee schedule that applied, chases the records and the provider's documents on the clock, routes the decision to provider services or the claims team in the Decision Desk with the comparison and only the allowed outcomes, and writes the decision and any adjustment to the claims system with the written response to the provider inside the required period.

The problem today

The provider's dispute letter listed forty claims; the plan answered twelve, and the rest became a complaint to the insurance department.

Today, without Callvu

Providers dispute payments in bulk: a spreadsheet of claims, a letter citing the contract, a fax. The plan's provider services team opens a ticket, asks claims for the adjudication, asks contracting for the rate, and answers what it can before the deadline. The disputes that were right cost the plan interest and penalties; the ones that were wrong cost the provider time and the relationship.

The contract sets the dispute process and its windows; the state's prompt pay and provider dispute laws set the period for a written determination (forty-five working days in some states) and the interest and penalties for a late or wrong payment; the plan's own rules set who may decide what. Each dispute is a comparison between what the contract says and what the claims system paid, and the comparison can be made by the process before a person looks.

A dispute with the contract rate, the adjudication and the records side by side is a dispute a person can decide in minutes.

How it runs

From the requestto the system of record.

A provider submits a dispute through the provider portal link, a letter, a fax or a call. Callvu opens the case with each claim disputed and the receipt date.

Provider
Submits the dispute with the claims, the reason and the documents through their link, and receives the written determination.
Provider services
Owns the case and decides contract and administrative disputes in the Decision Desk.
Claims team
Decides adjudication and coding disputes in the Decision Desk and processes adjustments.
Callvu
Captures, classifies, checks the window, prefills the comparison, chases, routes, writes and responds.
  1. Capture and classify

The dispute is captured through the provider's link with each claim number, the amount disputed and the reason, or converted from a letter or fax with the claims matched; each claim is classified: an underpayment against the contract rate or fee schedule, a denial for medical necessity or a missing authorization, a timely filing, coding, bundling or modifier edit, a duplicate or a coordination of benefits question; the dispute window under the contract and the state's rule is checked from the remittance date, and the acknowledgment goes to the provider.

  2. Prefill the comparison, chase and verify

For each claim the adjudication (what was paid and why, the edits applied, the authorization on file), the contract's terms and the fee schedule in effect on the date of service, and the expected payment under them are prefilled side by side; the records the dispute type needs (clinical records for a medical necessity denial, the authorization reference, the corrected claim, the primary payer's remittance for coordination of benefits) are requested from the provider or the plan's own systems and chased on the clock; each is read and checked against the claim.

  3. Decide

A claim where the comparison shows a clear underpayment or a clear correct payment is decided by rule where the plan's policy allows; the rest go to provider services (contract and administrative) or the claims team (adjudication and coding) in the Decision Desk with the comparison, the records and the reason, and only the allowed outcomes: uphold with the specific reason and the contract or policy provision, overturn with the adjustment, or partially overturn; a medical necessity dispute goes to the medical reviewer.

  4. Write back and respond

The decision and any adjustment with the interest the state requires are written to the claims system, the adjusted remittance follows, the written determination with the reason and the next step (the contract's escalation, the state's independent review or complaint process) goes to the provider inside the required period, and the case with every date is written to the system of record from which the state's dispute reporting is produced.

Where it ends: Each disputed claim was compared with the contract and the adjudication before a person looked, decided inside the window with the reason on the record, and the claims system holds the adjustment and the provider received the determination on time. The Decision Trace shows the classification, the comparison and the decision; the Audit Trace shows the receipt, the acknowledgment, the chases, the decision and the response with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

The provider portal link, or a letter or fax converted into the case with the claims matched.

The back office

Provider services and the claims team decide from a comparison, claim by claim; contracting is not asked for the rate by email.

The Decision Desk

  • A contract or administrative dispute goes to provider services, an adjudication or coding dispute to the claims team, and a medical necessity dispute to the medical reviewer, each in the Decision Desk with the comparison and the records and only uphold with the provision, overturn with the adjustment, or partially overturn.
