Member appeals and grievances

How do we take a member's appeal or grievance from a call, a letter or the portal, classify it correctly, collect the documents, and decide it within the Medicare Advantage, Medicaid or ACA timeframes?

Classify the case on arrival and let the classification set the clock. When a member or their representative contacts the plan with a complaint, Callvu captures it on a controlled screen, classifies it (an appeal of a coverage or payment decision versus a grievance about service or quality; pre-service or post-service; standard or expedited; the line of business, which sets the regulatory timeframes), confirms the representative's authority, acknowledges in writing, chases the records and the provider's documents on the clock, routes the decision to the appeals and grievances team or the medical director in the Decision Desk with the file and only the allowed outcomes, generates the decision letter with the reasons and the next-level rights, and writes the case to the system of record with every date the regulator measures.

The problem today

The letter was logged as a grievance, so nobody started the thirty-day appeal clock, and the plan reported a late decision to the regulator.

Today, without Callvu

A member writes to say their claim was denied and the clinic was rude. The letter contains an appeal and a grievance; the intake logs one. The medical director sees the file on day twenty-five with no records attached. The decision letter is a template that does not state the reason. The regulator's audit counts the late and the incomplete cases, and the plan's rating pays for it.

Each line of business has its own definitions and clocks: Medicare Advantage organization determinations and reconsiderations (standard pre-service in thirty days, expedited in seventy-two hours, payment in sixty), Medicaid managed care appeals and grievances (appeals in thirty days, expedited in seventy-two hours, grievances in the state's period), ACA individual and group internal appeals (pre-service in thirty days, post-service in sixty, urgent in seventy-two hours) with external review after. The classification is the compliance.

A case classified correctly on arrival, with its clock and its reviewer set by rule, is a case the regulator will find on time.

How it runs

From the requestto the system of record.

A member, a representative or a provider on the member's behalf contacts the plan with a complaint about a decision or a service, by call, letter, fax, the portal or an AI assistant. Callvu opens the case with the receipt time.

Member
States the complaint, names a representative where one acts, supplies documents through a link and receives the acknowledgment and the decision.
Appeals and grievances team
Owns the case, assembles the file and decides grievances and non-clinical appeals in the Decision Desk.
Medical director
Decides clinical appeals in the Decision Desk as a reviewer not involved in the original decision.
Callvu
Captures, classifies, acknowledges, chases, routes, generates the letters, tracks the clocks and writes back.
  1. Capture and classify

The complaint is captured with the member, the decision or event complained of, the date and the request; it is classified into one or more cases: an appeal (of a coverage denial, a payment denial, a reduction or termination of a service) or a grievance (about access, quality, a staff interaction, a privacy concern), pre-service or post-service, standard or expedited (where the standard time could seriously jeopardize the member's life, health or ability to regain function, or where a physician requests it), and the line of business; the classification sets the regulatory clock, the reviewer and the required letters; a representative's appointment form or authority is confirmed.

  2. Acknowledge, chase and verify

The written acknowledgment goes out within the line's period with the case number and the process; the records the decision needs (the original determination and its basis, the clinical records from the provider, the member's documents, the plan's policy) are requested from the holders through links and chased on the clock, read and checked on arrival, with the member told what has been requested; a continuation of benefits pending the appeal is applied where the rules require it.

  3. Decide

A clinical appeal goes to the medical director (a physician with the relevant expertise who did not make the original decision) in the Decision Desk with the file, the criteria and the member's statement, and only the allowed outcomes: overturn, uphold with the specific reasons and criteria, or partially overturn; a grievance or a non-clinical appeal goes to the appeals and grievances team with the same structure; a case that cannot be decided in time triggers the extension notice where the rules allow one, or an automatic forwarding to the independent review entity for Medicare Advantage.

  4. Notify and write back

The decision letter is generated from the decision with the reasons in plain language, the clinical rationale and criteria for an adverse decision, and the next-level rights (the independent review entity, the state fair hearing, external review, the ombudsman); an overturned decision is effected (the authorization issued, the claim paid) within the rule's period; the case with every date (receipt, acknowledgment, requests, decision, effectuation, letters) is written to the system of record, from which the regulatory reporting and the audit universes are produced.

Where it ends: Every complaint became the right case with the right clock on arrival, the file was assembled by chases rather than by the reviewer, the decision was made by the right person with the reasons on the record, and the system of record holds the dates the regulator measures. The Decision Trace shows the classification, the reviewer and the decision; the Audit Trace shows the receipt, the acknowledgment, each request, the decision and the effectuation with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

A call, a letter, a fax, the portal or an AI assistant that hands the member to the controlled screen; documents through a link.

The back office

The appeals and grievances team works a classified case with the file assembled; the medical director decides from the file, never from a day-twenty-five handoff.

The Decision Desk

  • A clinical appeal goes to the medical director in the Decision Desk with the file and the criteria and only overturn, uphold with reasons, or partially overturn; a grievance or a non-clinical appeal goes to the appeals and grievances team with the same outcomes and the extension or forwarding where the rules require.

Questions

What buyers ask next,answered plainly.

What is the difference between an appeal and a grievance?

An appeal challenges a coverage, payment or service decision and carries the regulatory decision clock; a grievance is a complaint about service, access or quality with its own period. A letter containing both becomes two cases.

When is an appeal expedited?

When the standard time could seriously jeopardize the member's life, health or ability to regain function, or a physician requests it; the decision is then due within seventy-two hours.

Who decides a clinical appeal?

A physician with the relevant expertise who was not involved in the original decision, in the Decision Desk with the file and the criteria, with the reasons in the letter.
