EHR access requests

How do we provision EHR access for staff only after the role, the approvals and the training completion are checked, and record the grant?

Make access a decision with evidence, not a ticket. When staff need system access (a new hire, a role change, a contractor, a student), Callvu prefills the request from the HR system (the person, the role, the department, the employment status, the start and end dates), classifies the access the role is entitled to under the role-based access model (the EHR modules, the patient population, the privileges, the break-the-glass rights), checks the prerequisites (the privacy and security training completed, the confidentiality agreement signed, the credentials verified for a clinical role, the sanctions screening clear), routes the request to the manager and the data owner in the Decision Desk with the entitlement and the evidence, provisions the approved access through the identity system with the end date, and records the grant with who approved it for the access review. The problem today

The contractor's EHR access was granted from an email, never ended, and showed up in the breach investigation two years later.

Today, without Callvu

Access to the EHR is the hospital's most sensitive control and the one most often granted by convenience: a manager's email, a ticket copied from a colleague's access, a contractor who got the same rights as an employee, a student whose access outlived the rotation. The access review finds hundreds of accounts nobody can explain, and the breach investigation finds the one that mattered.

The Security Rule requires access authorization and establishment procedures, workforce clearance, termination procedures and periodic reviews; the hospital's role-based access model defines what each role may see and do; the training and the confidentiality agreement are prerequisites; the data owner and the manager approve; the identity system provisions with an end date. Each is a check the process can run before anyone gets a login.

Access granted on evidence, with an end date, is access the review can explain.

How it runs

From the requestto the system of record.

A manager requests access for a person, or the onboarding or a role change triggers it. Callvu opens the request from the HR system record.

Requesting manager
Confirms the role and the need, and approves in the Decision Desk.
Data owner or security
Approves access outside the standard role entitlement in the Decision Desk.
The person
Completes the training and signs the confidentiality agreement through their link where not yet done.
Callvu
Prefills, classifies the entitlement, checks the prerequisites, routes, provisions and records.
  1. Prefill and classify

The request is prefilled from the HR system (the person, the role, the department, the employment type, the status, the start and end dates, the manager); the role-based access model classifies the standard entitlement (the EHR modules and functions, the patient population or units, the order and documentation privileges, the break-the-glass rights) and flags any request beyond it as an exception needing the data owner.

  2. Check the prerequisites

The privacy and security training completion, the signed confidentiality agreement, the verified credentials for a clinical role (from the onboarding or credentialing processes), the sanctions screening result and the active employment status are checked; a missing prerequisite is chased from the person or the process that owns it, and the request waits.

  3. Decide

The manager approves the standard entitlement in the Decision Desk with the evidence; an exception beyond the role, a contractor or a student, a request for a sensitive unit or function (behavioral health, employee records, research data), or a break-the-glass expansion goes to the data owner or security in the Decision Desk with the entitlement and the evidence and only the allowed outcomes (approve as requested, approve a reduced scope, approve with an end date, or deny with the reason).

  4. Provision and write back

The approved access is provisioned through the identity system with the roles, the scope and the end date (the contract end, the rotation end, a review date), the grant is recorded with the request, the approvals, the evidence and the dates for the periodic access review, the person is told what they have and the rules, and a role change or a termination from the HR system triggers the same process to modify or remove the access on the day.

Where it ends: Access was granted to the role's entitlement, after the training and the agreement, approved by the manager and the data owner where required, provisioned with an end date, and recorded so the review and the termination can act on it. The Decision Trace shows the entitlement, the prerequisite checks and the approvals; the Audit Trace shows the request, the provisioning and the end date with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

The manager's request, the onboarding or a role change from the HR system; the person completes prerequisites through their link.

The back office

The manager and the data owner approve with evidence; the identity system provisions; security reviews from the record.

The Decision Desk

  • The manager approves the standard entitlement; an exception beyond the role, a non-employee, a sensitive unit or function, or a break-the-glass expansion goes to the data owner or security in the Decision Desk with the evidence and only approve, approve reduced, approve with an end date, or deny with the reason.

Each result is a published claim, with its code on agents.callvu.com, where its status, source and last verified date are kept.

Every run produces a Decision Trace (why each AI action was allowed) and every workflow execution an Audit Trace (the chronological governance record).

Questions

What buyers ask next,answered plainly.

What is checked before access is granted?

The role's entitlement under the access model, the privacy and security training, the signed confidentiality agreement, the verified credentials for a clinical role, the sanctions screening and the active employment status.

Who approves?

The manager for the standard entitlement; the data owner or security for anything beyond the role, for non-employees, for sensitive units or functions, and for break-the-glass expansions.

How is access ended?

Every non-employee grant has an end date, and a termination or role change in the HR system triggers removal or modification the same day, recorded for the access review.
