Surgical informed consent, chained with anesthesia consent

How do we take the surgical informed consent, chained with the anesthesia consent, on a controlled screen ahead of the day with the identity checked and the signed record filed to the EHR?

Run consent as a chain the patient completes after the consult, not on the gurney. After the surgical consult, Callvu prefills the consent from the EHR and the booking (the patient, the procedure as scheduled, the surgeon, the site and laterality, the anesthesia plan), presents the surgical consent with the procedure-specific content (the risks, the benefits, the alternatives, the questions the patient asked and the answers) on a controlled screen in the patient's language, checks the identity of the signer and their authority (the patient, the legal representative, the guardian), captures the signature and the witness where required, chains the anesthesia consent with its content and signature, chases an unsigned consent on the surgery-date clock, routes a question, a refusal or a capacity concern to the surgeon or anesthesia in the Decision Desk, and files the signed records to the EHR for the pre-procedure verification. The problem today

The consent was signed in pre-op by a patient who had already received a sedative, for a procedure described in a different word than the schedule.

Today, without Callvu

Informed consent is the legal and ethical foundation of the operation, and it is often the most casually handled document: a form signed in the holding area under time pressure, a procedure name that does not match the schedule, a laterality missing, a witness line blank, a patient who asked a question nobody wrote down. When something goes wrong, the consent is the first document examined.

The rules are specific: the content (the procedure, the risks, the benefits, the alternatives, the practitioners), the person who may sign (the patient with capacity, or the legal representative), the timing (before sedation, with time to ask), the witness and the language access, the anesthesia consent as a separate discussion, and the record in the EHR the pre-procedure verification checks. A consent chained from the consult, completed on the patient's own screen with the identity checked, meets all of it before the day.

Consent is a conversation with a record; the record should match the conversation and the schedule.

How it runs

From the requestto the system of record.

The surgical consult concludes and the procedure is booked. Callvu opens the consent chain from the booking.

Patient or legal representative
Reads the consent in their language, asks questions, signs on a controlled screen with identity verified.
Surgeon and anesthesia practitioner
Confirm the content, answer questions, and decide refusals and capacity concerns in the Decision Desk.
Witness and interpreter
Attest where the policy or the state requires, recorded in the chain.
Callvu
Prefills, presents, checks identity and authority, captures signatures, chains, chases, routes and files.
  1. Prefill and present the surgical consent

The consent is prefilled from the EHR and the booking (the patient's identity, the procedure as scheduled with the same terminology, the site and laterality, the surgeon and the practitioners who may participate, the anesthesia plan); the procedure-specific content (the risks, the benefits, the alternatives, the blood product and the tissue provisions where applicable) is presented in the patient's language with the interpreter engaged where needed, and the patient's questions and the answers are recorded.

  2. Chase and verify the signer and sign

The signer's identity is verified and their authority classified (the patient with capacity, a legal representative with the document, a parent or guardian for a minor, a court order where applicable); the signature is captured on the controlled screen with the date and time, the witness attestation where the policy requires, and the interpreter's attestation; a consent not completed is chased on the surgery-date clock.

  3. Chain the anesthesia consent and decide exceptions

The anesthesia consent with the plan's content (the type of anesthesia, the risks, the alternatives, the practitioners) is presented and signed the same way, chained from the surgical consent's completion; a question the surgeon must answer, a refusal, a change in the procedure after signing, or a capacity concern goes to the surgeon or anesthesia in the Decision Desk with the record and only the allowed outcomes (answer and continue, re-consent with the changed content, assess capacity, or cancel).

  4. File and write back

The signed consents with the content presented, the questions and answers, the signer's identity and authority, the witness and interpreter attestations and the timestamps are filed to the EHR against the procedure, and the pre-procedure verification reads them; a procedure change after signing invalidates the consent and starts a re-consent.

Where it ends: The patient consented after the consult, with time, in their language, on a record that matches the schedule, with the identity and authority checked and the anesthesia consent chained, all filed in the EHR before the day. The Decision Trace shows the content presented, the identity check and any decision; the Audit Trace shows each signature and attestation with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

The link after the consult; the patient or representative reads and signs on their own screen with an interpreter where needed; a controlled tablet in the clinic as the fallback.

The back office

The surgeon and anesthesia answer questions and decide exceptions; the pre-procedure verification reads the EHR.

The Decision Desk

  • A question the surgeon must answer, a refusal, a procedure change after signing or a capacity concern goes to the surgeon or anesthesia in the Decision Desk with the record and only answer and continue, re-consent, assess capacity, or cancel.

At the Ministry of Health, a process chains up to five patient forms and signatures and ten internal steps; completing one step starts the next, for example surgery consent starts anesthesia consent and sends the procedure to the approving committee.

At the Ministry of Health, Callvu integrated with very old legacy systems at several hospitals, and brings ERP, CRM and EHR data into one experience.

Questions

What buyers ask next,answered plainly.

When is the consent signed?

After the consult and before the day, on the patient's own screen with time to ask questions, never after sedation; it is chased on the surgery-date clock.

How does the consent match the schedule?

The procedure, the site and laterality and the practitioners are prefilled from the booking, and a change after signing starts a re-consent.

Who can sign?

The patient with capacity, or the legal representative, guardian or parent with their authority verified and recorded; a capacity concern goes to the practitioner.
