Procedure and treatment consents by type
How do we get the right consent form signed ahead of time for each ordered test, infusion, imaging with contrast or other procedure, and filed to the EHR before the patient arrives?
Let the order choose the form. When a test, an infusion, an imaging study with contrast, a transfusion or another procedure is ordered in the EHR, Callvu classifies the order to the consent the policy requires (the form, the version, the language, whether a clinician's discussion must be documented, whether a witness is needed), prefills the patient, the procedure and the ordering clinician, delivers the consent to the patient's phone ahead of the appointment with the explanation and the risks the form carries, captures the signature on a secure screen after the questions are answered, routes a consent that needs a clinician's discussion or a representative's authority to the right person in the Decision Desk, and writes the signed consent to the EHR against the order so the day-of check finds it.

The problem today
The contrast consent was signed on the table, in a gown, with a pen that did not work, because the order and the form lived in different systems.
Every ordered procedure with risk needs a consent: a biopsy, an infusion, a CT with contrast, a transfusion, a sedation. The order is in the EHR; the consent forms are in a document library; the technologist decides which one applies and gets it signed at the moment of the procedure, when the patient cannot read it and the clinician who should discuss it is elsewhere. Missing or wrong consents are found by the day-of check or by the auditor.
Informed consent rests on the hospital's conditions of participation and the state's informed consent law (the disclosure of the procedure, the risks, the benefits and the alternatives by the responsible practitioner, documented), on the accreditor's standards for the consent's contents, and on the privacy rule for the record. The right form, version and language are a classification of the order; the signature is a captured event; the record belongs against the order in the EHR.
A consent signed at home, after reading it, is a better consent and a complete record.
How it runs
From the requestto the system of record.
A procedure that requires consent is ordered in the EHR. Callvu classifies the order and opens the consent.
- Patient or personal representative
- Reads the consent, asks questions, and signs on a secure screen ahead of the appointment.
- Ordering or performing clinician
- Documents the informed consent discussion where the policy requires and answers questions.
- Department staff
- See consent status before the appointment and decide gaps in the Decision Desk.
- Callvu
- Classifies the order to the form, prefills, delivers, captures the signature, routes and writes.
Classify the order to the consent
The procedure code, the department, the agents involved (contrast, sedation, blood products, a research or investigational element) and the patient's age and status classify the consent form and version the policy requires, the language, whether a documented clinician discussion is required, whether a witness or an interpreter is needed, and who may sign (the patient, a parent, a guardian, a health care agent).
Prefill and deliver
The consent is prefilled with the patient, the procedure as ordered, the performing clinician and the scheduled date, and delivered to the patient's phone ahead of the appointment in their language with the explanation, the risks, the benefits and the alternatives the form carries, and a way to ask questions that reach the clinician.
Capture and decide
The patient signs on a secure screen after confirming they read the form and had their questions answered, with the witness and interpreter attestations where required; a consent that requires a documented clinician discussion before signature, a representative whose authority is not on file, a minor under the state's rules, or a patient who declines goes to the clinician or the department in the Decision Desk with only the allowed outcomes (complete the discussion and re-present, verify the representative, obtain the consent at the visit, or cancel the order).
Write back
The signed consent with the form version, the signers, the witness, the interpreter and the discussion documentation is written to the EHR against the order, the day-of readiness check reads it, and an order change that alters the procedure re-opens the consent.
Where it ends: Every ordered procedure had the right consent form in the right language signed ahead of time by the right person, with the discussion documented where required and the record filed against the order in the EHR. The Decision Trace shows the classification and any decision; the Audit Trace shows the delivery, the questions and the signature with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The patient's phone ahead of the appointment; the check-in screen as the fallback.
The back office
Department staff see consent status instead of chasing clipboards; the day-of check reads the EHR.
The Decision Desk
- A consent needing a documented clinician discussion, an unverified representative, a minor or a decline goes to the clinician or the department in the Decision Desk with only complete the discussion, verify the representative, obtain at the visit, or cancel the order.

Questions
What buyers ask next,answered plainly.
Which form is used?
The one the order classifies to: the procedure, the department, the agents involved and the patient's status choose the form, the version and the language under the policy.
Can the patient sign before the appointment?
Yes, on their phone after reading the form and asking questions; a consent that needs the clinician's discussion first waits for it to be documented.
Where is the signed consent?
In the EHR against the order, where the day-of readiness check finds it.
