Outside results and records intake
How do we take outside labs, imaging and notes that arrive by fax, portal or exchange, match them to the right patient, and route them to the clinician for review and acknowledgment?
Treat every outside document as a classified, matched, routed item. When outside results or records arrive (a fax, a portal upload, a health information exchange message, a mailed report, a patient's own upload), Callvu reads each document and classifies it (a lab result, an imaging report, a consult note, a discharge summary, a pathology report, a record for a referral), matches it to the patient and the ordering or treating clinician using the identifiers and the chart, flags a critical or abnormal result, chases the sender for a missing page, an unreadable result or an unmatched patient, routes the document to the clinician's inbox for review and acknowledgment with the critical ones escalated on the policy's clock, routes an unmatched or disputed document to health information management in the Decision Desk, and writes the document to the EHR against the patient with the acknowledgment recorded.

The problem today
The abnormal biopsy result from the outside lab sat in the fax queue for nine days under a slightly different spelling of the patient's name.
Outside results come in every channel and format: the fax machine, a payer or hospital portal, an exchange message, a patient's photo of a report. Someone in medical records indexes them to the chart by hand, the clinician's inbox fills with scanned pages, and a critical result waits for the clinician who happens to open it. Delayed or missed results are among the most common causes of diagnostic harm and of malpractice claims.
The Privacy Rule governs the records as protected health information; the accreditor's patient safety goal requires critical results to reach the responsible clinician within the organization's defined time; the clinical laboratory rules require critical values to be reported; the information blocking rules require results to reach the patient promptly. Each document has a type, a patient, a clinician and a priority, and the match and the routing can be done by rule with a person deciding only the exceptions.
A result matched and routed the day it arrives, with the critical ones escalated, is a result that is acted on.
How it runs
From the requestto the system of record.
An outside document arrives by fax, portal, exchange, mail or patient upload. Callvu reads it and opens the intake.
- Sender
- An outside lab, imaging center, hospital or practice that sent the document and answers chases for missing pages or identifiers.
- Clinician
- Reviews and acknowledges the document in their inbox and acts on flagged results.
- Health information management
- Decides unmatched and disputed documents in the Decision Desk.
- Callvu
- Reads, classifies, matches, flags, chases, routes, escalates and writes.
Read and classify
Each document is read on arrival (the fax pages, the uploaded file, the exchange message) and classified by type (a lab result, an imaging report, a pathology report, a consult note, a discharge summary, a record for a referral or a prior authorization), with the sender, the date of service and the result values or impression extracted where structured, and an abnormal or critical flag set from the values and the reference ranges or the report's language.
Match the patient and the clinician
The document is matched to the patient using the identifiers on it (the name, the date of birth, the medical record number, the ordering clinician, the order on file) against the chart with a confidence rule, and to the ordering or treating clinician from the order or the care team; a match below the confidence threshold, a patient not found, or a document for a patient with no relationship is not filed.
Chase and verify, and route
A missing page, an unreadable result or a missing identifier is chased from the sender with the specifics; a matched document is routed to the clinician's inbox for review and acknowledgment with the flag, a critical result is escalated on the policy's clock (to the clinician by page or call, then to the covering clinician) until acknowledged; an unmatched document, a disputed match, or a document that appears to be for another organization goes to health information management in the Decision Desk with only the allowed outcomes (file to the matched patient, request the sender's correction, return the document, or hold for identification).
Write back
The document is written to the EHR against the patient and the order with its classification and the extracted values, the clinician's acknowledgment and any action are recorded, the result is released to the patient's portal under the information blocking rules, and the intake log shows every document by channel, status and age.
Where it ends: Every outside document was classified, matched by rule, routed to the right clinician with critical results escalated until acknowledged, and the EHR holds it against the patient with the acknowledgment. The Decision Trace shows the classification, the match confidence and any decision; the Audit Trace shows the arrival, the routing, the escalation and the acknowledgment with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The fax line, the portals, the exchange, the mail scan and the patient's upload; the sender's link for chases.
The back office
Clinicians receive matched, flagged documents; health information management decides only exceptions from the Decision Desk.
The Decision Desk
- An unmatched document, a disputed match or a document for another organization goes to health information management in the Decision Desk with only file to the matched patient, request the sender's correction, return, or hold for identification.

Questions
What buyers ask next,answered plainly.
How is a faxed result matched to the patient?
By the identifiers on it against the chart and the order on file, with a confidence rule; a match below the threshold is decided by health information management, never filed by guess.
What happens to a critical result?
It is flagged on arrival and escalated to the responsible clinician on the organization's defined clock, then to the covering clinician, until it is acknowledged.
When does the patient see it?
Promptly after filing to the EHR, through the portal, under the information blocking rules.
