Payer document requests
How do we answer a payer's request for more records by matching it to the case, pulling and checking the documents, and sending the response on the payer's clock?
Treat each payer request as a case with a deadline and a specific list. When a payer asks for additional documentation (on a pended claim, a pre-payment review, a post-payment audit, an authorization), Callvu reads the request and matches it to the patient, the encounter, the claim and the authorization, classifies the request type and the payer's deadline, lists the specific records asked for, pulls them from the EHR and the billing system (or chases a department for what is not filed), checks each against the request (the dates of service, the record type, the completeness, the signatures), masks what the minimum necessary standard excludes, routes an ambiguous request or a record that cannot be found to the revenue integrity or the health information team in the Decision Desk, and sends the response through the payer's channel with what was sent logged against the claim. The problem today
The payer's request was answered on day thirty-one with the wrong encounter's notes, and the claim denied for untimely documentation.
Payers ask for records constantly: a pended claim wants the operative note, a pre-payment review wants the whole chart, an audit wants a sample of encounters, an authorization wants the clinical criteria evidence. The letters arrive in the mailroom, the fax queue or a portal; someone matches them to a claim by eye; the records are pulled by a department that has forty other requests; the response goes out late or incomplete, and the claim denies or the audit extrapolates.
Each request has a type, a deadline the payer or the regulation sets, a specific list of records, and a channel for the response. The records are in the EHR and the billing system; the minimum necessary standard limits what is sent; the response must be logged because the appeal will depend on proving what was sent and when. The matching and the pulling are rules; the deadline is a clock.
A payer request answered on time with exactly what was asked is a claim that pays.
How it runs
From the requestto the system of record.
A payer's request for records arrives by letter, fax, portal or electronic attachment request. Callvu reads it and opens the case.
- Payer
- Sends the request and receives the response through its channel.
- Health information management and clinical departments
- Supply records not filed in the EHR through their links.
- Revenue integrity or health information team
- Decides ambiguous requests and unfound records in the Decision Desk.
- Callvu
- Reads, matches, classifies, lists, pulls, chases, checks, masks, routes, sends and logs.
Classify and match
The request is read (the payer, the patient identifiers, the dates of service, the claim or authorization reference, the records requested, the deadline, the response channel) and matched to the patient, the encounter, the claim and any authorization; the request type (pended claim, pre-payment review, post-payment audit, authorization support, coordination of benefits) sets the deadline and the rules.
Chase and verify the records
The specific records are listed and pulled from the EHR and the billing system (the notes, the operative report, the orders, the medication administration record, the itemized bill, the authorization); a record not filed is chased from the department through its link with the specifics; each is checked against the request: the right patient and dates of service, the record type asked for, the completeness, the signatures and the authentication; the minimum necessary standard is applied and the content outside the request is masked.
Decide
A complete, matched set proceeds by rule; an ambiguous request (dates that match two encounters, a record type the hospital does not keep), a record that cannot be found, a request that exceeds the minimum necessary for its purpose, or an audit sample above the policy's threshold goes to the revenue integrity or health information team in the Decision Desk with the case and only the allowed outcomes (send as assembled, send with an explanation, request clarification from the payer, or escalate to compliance).
Send and write back
The response is sent through the payer's channel (the electronic attachment transaction, the portal upload, or mail) before the deadline, the exact set sent is logged against the claim with the date, the method and the confirmation, and the claim's status is updated in the billing system so the follow-up and the appeal processes have the record.
Where it ends: Every payer request was matched to its case, answered with the specific records checked and masked, sent on the payer's clock, and logged against the claim with what went and when. The Decision Trace shows the match, the record checks and any decision; the Audit Trace shows the request, the chases and the response with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The payer's letter, fax, portal or electronic request, read on arrival; departments through their links.
The back office
The revenue integrity and health information teams decide only exceptions; the response and the log run from the case.
The Decision Desk
- An ambiguous request, an unfound record, a request beyond the minimum necessary or an audit sample above the threshold goes to the revenue integrity or health information team in the Decision Desk with the case and only send, send with explanation, request clarification, or escalate to compliance.

Questions
What buyers ask next,answered plainly.
How is the request matched?
By reading the payer, the patient identifiers, the dates of service and the claim or authorization reference and matching them to the patient, the encounter and the claim; an ambiguous match goes to a person.
What is sent?
Only the records asked for, pulled from the EHR and the billing system, checked against the request and limited to the minimum necessary, through the payer's channel before the deadline.
Why log what was sent?
Because the appeal and the audit depend on proving exactly what went to the payer and when; the log is on the claim with the method and the confirmation.
