Authorization renewals and extensions
How do we renew or extend an authorization before it expires or the approved visits run out, with the updated notes collected and the request sent to the payer before the deadline?
Watch every authorization's end and start the renewal from it. Callvu tracks each active authorization's validity dates and remaining units from the EHR, classifies what the payer requires to renew that service (updated progress notes, a new order, a treatment plan with goals, measured outcomes, the submission channel and the lead time), opens the renewal at the lead time before the expiry or the last unit, chases the updated documentation from the treating clinician and checks it against the payer's criteria, submits the renewal, tracks the decision on the payer's timeframe, routes a denial or a reduction to the clinician in the Decision Desk, and writes the new validity and units to the EHR so scheduling and billing continue without a gap.

The problem today
The physical therapy authorization ran out at visit twelve, and visits thirteen through sixteen were written off.
Authorizations have ends: a date, a number of visits, a number of units. Care usually continues past them. The renewal depends on someone noticing the end, asking the clinician for updated notes, and sending the request far enough ahead of the expiry for the payer to decide. In most departments the notice comes from a denied claim.
Each payer sets what a renewal needs (progress notes showing improvement toward goals, a new plan of care, a physician's signature, measured outcomes), the lead time it wants, and the decision timeframe; the clinical documentation requirements for continued therapy, infusions, home health or behavioral health are specific and audited. The tracking is arithmetic on the authorization record; the chase is the clinician's documentation; the clock is the payer's.
A renewal that starts at the lead time, not at the denial, keeps the care and the revenue continuous.
How it runs
From the requestto the system of record.
An active authorization reaches its renewal lead time (by date or by remaining units), or a clinician extends the plan of care. Callvu opens the renewal on the authorization.
- Treating clinician
- Supplies the updated notes, the plan of care and the outcomes, and decides on a denial or a reduction in the Decision Desk.
- Authorization team
- Sees renewals by expiry date and handles payer exceptions.
- Payer
- Receives the renewal and returns the decision.
- Callvu
- Tracks validity and units, classifies the renewal requirements, chases and checks the documentation, submits, tracks, routes and writes back.
Track and classify
Each active authorization's validity dates, approved units and used units are read from the EHR and the scheduling record; the payer and the service classify the renewal requirements (the documents, the signatures, the channel, the lead time, the decision timeframe) and the renewal opens at the lead time before the expiry date or the last unit, whichever comes first.
Chase and verify the documentation
The updated documentation is requested from the treating clinician through their inbox task with the specifics the payer's criteria name (progress toward the goals, the objective measures, the continued medical necessity, the revised plan of care with dates and frequency, the physician's signature); each document is read on arrival and checked against the criteria, and a gap is sent back with the specifics before the request goes.
Submit, track and decide
The renewal is submitted through the payer's channel with the documents and the current authorization number, tracked on the payer's timeframe with pends answered and late decisions escalated; an approval proceeds by rule; a denial, a reduction in units or frequency, or a request for a peer-to-peer goes to the treating clinician in the Decision Desk with the payer's reasons and only the allowed outcomes (accept, request the peer-to-peer, appeal, revise the plan, or discharge from the service).
Write back
The new validity dates, units and any conditions are written to the EHR against the authorization and the plan of care, scheduling continues against the new units, billing carries the renewed number on the claims, and the next renewal lead time is set from the new end.
Where it ends: No authorization lapsed unnoticed: each renewal opened at the lead time, carried the documentation the payer's criteria require, was tracked on the payer's clock, and the EHR holds the new validity so care and billing continued. The Decision Trace shows the tracking, the requirements and any decision; the Audit Trace shows the chases, the submission and the write with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
None for the patient; the clinician's inbox task for the documentation.
The back office
The authorization team sees renewals by expiry and handles payer exceptions; scheduling and billing read the EHR.
The Decision Desk
- A denial, a reduction in units or frequency, or a peer-to-peer offer goes to the treating clinician in the Decision Desk with the payer's reasons and only accept, request the peer-to-peer, appeal, revise the plan, or discharge from the service.

Questions
What buyers ask next,answered plainly.
When does the renewal start?
At the payer's lead time before the expiry date or before the last approved unit is used, computed from the authorization record and the scheduling data.
What does the clinician have to provide?
The specifics the payer's criteria name: progress toward goals with objective measures, continued medical necessity, the revised plan of care with dates and frequency, and the physician's signature, requested once through their inbox task.
What if the payer reduces the units?
The clinician decides in the Decision Desk with the payer's reasons: accept, request the peer-to-peer, appeal within the deadline, revise the plan, or discharge.
