Sworn proof of loss

How do we get a sworn proof of loss signed and notarized by the claimant and filed to the claim before the state's or the policy's deadline?

Generate the proof of loss from the claim and chase the signature on the deadline. When the adjuster requests it, Callvu generates the sworn statement prefilled from the claim (the policy, the date and cause of loss, the interest of the insured and others, the encumbrances, the amount claimed from the inventory and estimates), sends it to the claimant through a link with the deadline and the instructions, arranges the notarization (remote online notarization where the state allows it, or a notary appointment), reads the returned statement and checks it against the claim, routes a variance to the adjuster in the Decision Desk, and files the executed proof to the claims system with the date received, which starts the state's decision clock.

The problem today

The proof of loss form was mailed blank; the claimant filled it in wrong, missed the sixty days, and the insurer's decision clock never started.

Today, without Callvu

The adjuster requests a sworn proof of loss. A blank form goes to the claimant by post with a cover letter. The claimant does not know what to put in the amount, cannot find a notary, and returns it late or not at all. The insurer cannot decide the claim without it, the claimant blames the insurer for the delay, and the state's decision period, which runs from receipt of the proof, has not begun.

The policy conditions require a signed, sworn proof of loss within sixty days of the insurer's request (or of the loss in some forms), with the time, place and cause of the loss, the interests of all parties, the encumbrances, other insurance, the changes in title and the inventory with values; several states set their own proof of loss deadlines and require the insurer to furnish the forms and instructions. Every field is already on the claim.

The proof of loss is a document the insurer can prepare and a signature the insurer can make easy.

How it runs

From the requestto the system of record.

The adjuster requests a sworn proof of loss on the claim, or the policy's condition requires one. Callvu generates it from the claim.

Claimant
Reviews the prefilled statement, corrects what is wrong, signs and has it notarized through the link.
Adjuster
Requests the proof, and decides variances between the statement and the claim in the Decision Desk.
Callvu
Generates, sends with the deadline, arranges notarization, reads and checks, routes and files.
  1. Generate from the claim

The sworn statement in proof of loss is prefilled from the claims system: the policy number and period, the insured and the mortgagee or other interests, the date, time, place and cause of the loss, the occupancy, the encumbrances, other insurance, the inventory or schedule of loss with the amounts from the documents on the claim, and the amount claimed; the form is the state's or the insurer's with the instructions the state requires.

  2. Chase and verify the signature

The claimant receives the statement through a link with the deadline (the policy's sixty days from the request or the state's period), the instructions and the notarization options: remote online notarization where the state and the policy allow it, or a notary appointment the process schedules; reminders go on the deadline's schedule; the returned statement is read, the signature and the notary's seal and commission are checked, and the figures are compared with the claim.

  3. Decide the variances

A statement that matches the claim is accepted by rule; an amount that differs from the documented loss, an interest or encumbrance not on the claim, or a cause of loss stated differently goes to the adjuster in the Decision Desk with the statement and the claim side by side and only the allowed outcomes: accept, request a corrected statement with the specific item, or reject with the reason in writing within the state's period.

  4. Write back

The executed proof of loss is filed to the claims system with the date received, which starts the state's decision clock; a rejection with reasons is sent within the state's period and recorded; the claimant can see that the proof was received and what happens next.

Where it ends: The claimant signed a statement that was already correct, notarized through the link, inside the deadline, and the claim's decision clock started on a recorded date. The Decision Trace shows the generation sources, the checks and any adjuster decision; the Audit Trace shows the request, the reminders, the notarization, the receipt and the filing.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

The claimant through the link, on a phone or a computer, with remote notarization where the state allows it.

The back office

The adjuster requests with one click and sees only variances; the claim's decision clock starts from the record.

The Decision Desk

  • A variance between the sworn statement and the claim (the amount, an interest, the cause) goes to the adjuster in the Decision Desk with both in view and only accept, request a corrected statement, or reject with reasons in writing within the state's period.

Questions

What buyers ask next,answered plainly.

What is in the proof of loss?

The time, place and cause of the loss, the interests and encumbrances, other insurance, the inventory or schedule with values and the amount claimed, all prefilled from the claim for the claimant to confirm and swear to.

How is it notarized?

By remote online notarization through the link where the state and the policy allow it, or by a notary appointment the process schedules.

What does the deadline do?

The claimant has the policy's sixty days or the state's period from the request; receipt of the proof starts the insurer's decision period, recorded on the claim.
