Hospital file completion
How do we complete a hospital stay's file by chasing the records the payment needs from the hospital, checked against the authorization and the claim, so the stay can be paid?
Treat each missing record as a chase against the hospital with a deadline. When a hospital stay needs a complete file for payment, Callvu classifies the stay (the admission, the authorization, the diagnosis and procedure codes, the discharge, the contract terms) and the records the payment rules require (the discharge summary, the operative and procedure reports, the itemized bill, the implant invoices, the physician orders, the admission and discharge dates), requests each from the hospital's records and billing departments through their links, chases on the clock the contract and the prompt pay rules set, reads each on arrival and checks it against the authorization and the claim (the dates, the codes, the units, the level of care), routes a mismatch or a medical necessity question to the reviewer in the Decision Desk, and writes the complete file to the health plan system so the claim pays. The problem today
The claim pended for seven weeks waiting for an operative report, while the hospital's records department waited for a request that had gone to billing.
A hospital stay generates a claim and a file: the authorization, the admission and discharge, the procedures, the itemized charges, the implants, the records that show the care was what was billed. The payer pends the claim for a missing record; the request goes to the wrong department; the hospital's revenue cycle team calls the payer's claims line; the stay is paid late, with the prompt pay interest, or denied for a document that existed all along.
Payment rests on the contract and the payer's rules: the authorization matching the stay, the level of care, the coding supported by the records, the itemized bill and the implant invoices, the discharge summary. The prompt pay rules set the periods for a clean claim and the interest for late payment. Every missing record has an owner at the hospital, and the chase should go to them with the specifics.
A complete file is the claim's cycle time, and the chase is where it is lost.
How it runs
From the requestto the system of record.
A hospital claim arrives or pends with the file incomplete, or a stay needs its file assembled for review. Callvu classifies the stay and opens one chase per record.
- Hospital records and billing departments
- Return the records and the documents through their links.
- Claims or clinical reviewer
- Decides mismatches and medical necessity questions in the Decision Desk.

Classify the stay and the records
The admission and discharge, the authorization and its terms, the diagnosis and procedure codes, the level of care, the contract's payment method and documentation requirements, and the prompt pay clock classify the stay and the records the payment needs; each record is mapped to the department that holds it.
Chase and verify
Each record is requested from its department through a link with the specifics (the patient, the dates, the record type, the claim reference), chased on the clock, read on arrival and checked against the authorization and the claim: the dates match, the procedures in the operative reports match the codes billed, the implant invoices support the charges, the physician orders support the level of care, the discharge summary is signed; what is missing or inconsistent is requested with the specifics.
Decide
A file that is complete and consistent proceeds to payment by rule; a mismatch between the records and the claim (a procedure billed without a report, a level of care the orders do not support, dates outside the authorization), a medical necessity question, or a hospital that does not respond goes to the claims or clinical reviewer in the Decision Desk with the file and only the allowed outcomes (pay, pay adjusted with the reason, deny with the reasons and the provider's appeal rights, or refer for audit).
Write back
The complete file and the decision are written to the health plan system, the claim is released for payment within the prompt pay period or adjusted or denied with the notice, the hospital is told the status through its link, and the file is retained for audit.
Where it ends: The stay's records were chased from the departments that hold them on a clock, checked against the authorization and the claim, and the health plan system holds a complete file that paid on time or was decided with reasons. The Decision Trace shows the records required, each check and any decision; the Audit Trace shows each request, chase and arrival with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The hospital's records and billing departments through their links; no calls to the claims line.
The back office
The reviewer opens a checked file and decides only mismatches; payment runs from the record inside the prompt pay period.
The Decision Desk
- A records-to-claim mismatch, a medical necessity question or a non-responsive hospital goes to the claims or clinical reviewer in the Decision Desk with the file and only pay, pay adjusted, deny with reasons and rights, or refer for audit.

Questions
What buyers ask next,answered plainly.
Which records are requested?
The ones the contract and the payment rules need for the stay: the discharge summary, the operative and procedure reports, the itemized bill, the implant invoices and the physician orders, each requested from the department that holds it with the specifics.
What is checked when a record arrives?
That the dates match the authorization and the claim, the procedures in the reports match the codes billed, the invoices support the charges and the orders support the level of care.
How does this affect prompt pay?
The pend notice names the specific records inside the period, the chases run on the contract's response times, and the claim releases for payment within the statutory period once the file is complete.
