Telehealth visit readiness

How do we capture the telehealth consent, the patient's location and contact details before a virtual visit, and send the visit link only when the visit is ready?

Make readiness a gate the visit link waits for. When a virtual visit is booked, Callvu prefills the patient's record and the visit from the EHR, chases the items the visit needs through a text to the patient's phone (the telehealth consent on the state's terms, the physical location the patient will be in at the visit, the phone number for a fallback, an emergency contact, the device and connection check, the payer's telehealth coverage and copay), verifies the location against the clinician's licensure and the payer's rules for the originating site, routes a location in another state or a failed coverage to the scheduling desk in the Decision Desk, and writes the consent, the location and the readiness to the EHR, releasing the visit link to the patient only when the visit is ready.

The problem today

The patient joined the video visit from a car in another state, and the clinician found out when they asked where the patient was.

Today, without Callvu

A virtual visit is booked and a link is emailed. On the day, the patient cannot find the link, the camera does not work, the clinician spends ten minutes on technical support, and nobody asked where the patient is: a clinician licensed in one state is treating a patient sitting in another. The consent the state requires was never captured; the payer's rules on the originating site were never checked.

Telehealth readiness is a short list the visit depends on: the consent the state's telehealth law requires in the form it requires, the patient's physical location at the visit (which decides the clinician's licensure and the payer's coverage), a fallback phone number for a dropped connection, the emergency contact and the nearest facility the clinician would need, a device and connection check, and the payer's telehealth coverage and copay. HIPAA sets the platform and the privacy of the visit. The link should go out when the list is done.

A visit link released by readiness is a visit that starts on time with a clinician who may lawfully give it.

How it runs

From the requestto the system of record.

A virtual visit is booked (through scheduling in any channel). Callvu opens the readiness case from the booking.

Patient or caregiver
Completes the consent, the location, the contacts and the device check on their phone.
Scheduling desk
Decides location, licensure and coverage questions in the Decision Desk.
Clinician
Sees the readiness, the location and the consent in the chart before the visit.
Callvu
Prefills, chases the items, verifies the location and coverage, routes, writes to the EHR and releases the link.
  1. Prefill and list the items

The patient's demographics, the contacts on file, the coverage and the clinician's licensure states, the visit type and the modality are prefilled from the EHR; the readiness items are listed by the state and the payer: the telehealth consent in the state's form, the location at the time of the visit, the fallback phone number, the emergency contact, the device and connection check, and the payer's telehealth coverage and copay.

  2. Chase and verify

A text sends the patient to a secure screen to complete the items; the consent is captured with the state's required elements (the nature of telehealth, the risks and limits, the right to refuse and to stop, the privacy of the session), the location is captured as an address and verified against the clinician's licensure states and the payer's originating site rules, the device and connection are tested, the coverage is checked with the payer for telehealth and the copay is shown (and collected through the deposit process where the policy requires); reminders follow until the items are done or a cutoff before the visit.

  3. Decide

A ready visit releases by rule; a location in a state where the clinician is not licensed, a payer that does not cover telehealth for the visit type, a failed device check the patient cannot resolve, or a consent refused goes to the scheduling desk in the Decision Desk with the item and only the allowed outcomes (reassign to a licensed clinician, convert to an in-person or audio-only visit where the rules allow, reschedule, or cancel with the reason).

  4. Write back and release

The consent, the location, the contacts, the device result and the coverage check are written to the EHR on the visit so the clinician sees them before joining, the visit link is released to the patient at the readiness or at the scheduled time with the fallback number on it, and the readiness status is on the scheduling record.

Where it ends: The visit started with the consent on file, the patient's location verified for licensure and coverage, a fallback number in hand and a tested device, and the EHR holds all of it on the visit. The Decision Trace shows the items, the verification and any decision; the Audit Trace shows the text, the completion, the checks and the link release with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

A text to the patient's phone from the booking; the items on a secure screen.

The back office

The scheduling desk sees only location, coverage and device exceptions; the clinician opens a ready visit.

The Decision Desk

  • A location outside the clinician's licensure, a payer that does not cover the virtual visit type, an unresolved device failure or a refused consent goes to the scheduling desk in the Decision Desk with the item and only reassign, convert the modality, reschedule, or cancel with the reason.

Questions

What buyers ask next,answered plainly.

Why ask where the patient will be?

Because the clinician must be licensed in the state where the patient is physically located during the visit, and the payer's coverage depends on the location; the address is captured and verified before the link is released.

What does the consent cover?

The state's required elements: the nature of telehealth, its risks and limits, the right to refuse or stop, and the privacy of the session, captured before the first visit and renewed as the state requires.

When does the patient get the link?

When the readiness items are done, or at the scheduled time if they finished earlier, with the fallback number on it.
