Patient cost estimates for insured patients

How do we give an insured patient an estimate of their share before a scheduled service, from their benefits and the contracted rate, and keep the estimate on the account?

Compute the share from the benefits and the contract, and deliver it before the visit. When a service is scheduled for an insured patient, Callvu prefills the patient, the service codes and the setting from scheduling, runs the eligibility and benefits check for the deductible met, the coinsurance, the copay and the out-of-pocket remaining, applies the payer's contracted rate for the service and the facility, computes the expected patient share with its assumptions stated, delivers the estimate in the patient's chosen form with the disclaimers, offers the payment and plan options, and writes the estimate with its version and delivery record to the billing system so the pre-service collection and the final bill refer to it.

The problem today

The patient was told 'it depends on your insurance' and found out what it depended on from a bill nine weeks later.

Today, without Callvu

Insured patients ask one question before a scheduled service: what will I owe? The answer exists in three places (the payer's benefits, the contracted rate and the charge master) and the front desk has none of them at hand. The patient arrives, pays a copay guessed from the card, and receives the real figure after the claim adjudicates, when the chance to plan or to pay had passed.

The price transparency rules require hospitals to publish standard charges and payer-specific negotiated rates and to offer a consumer tool; the payer transparency rules give members cost estimates from the plan's side; the No Surprises Act's convening provider rules will extend estimates to insured patients when the advanced explanation of benefits takes effect. In the meantime the hospital that computes the share from the benefits check and the contract collects earlier and disputes less.

An estimate before the visit is a payment decision the patient can make.

How it runs

From the requestto the system of record.

A service is scheduled for an insured patient, or an insured patient asks for an estimate. Callvu opens the estimate with the service and the coverage.

Patient
Receives the estimate in the chosen form, asks questions, and can pay or choose a plan from it.
Patient access and financial counseling
See the estimate and handle questions and exceptions.
Callvu
Prefills, runs eligibility, applies the contract, computes, delivers and writes.
  1. Prefill the service and the coverage

The patient, the scheduled service with its codes, the setting and the expected ancillaries are prefilled from scheduling and the EHR; the coverage on file is confirmed through the insurance verification process, and the benefits for the service type are retrieved: the deductible and how much is met, the coinsurance, the copay, the out-of-pocket maximum remaining, the network status of the facility and the providers, and any authorization requirement.

  2. Compute the share

The payer's contracted rate for the service and the setting (from the contract management system or the published negotiated rates) is applied, the benefits are applied to it in the plan's order, and the expected patient share is computed with the assumptions stated (the services assumed, the benefit figures as of the date, the exclusions); a service without a contracted rate or a benefit the payer could not return is flagged.

  3. Deliver and offer options

The estimate is delivered in the patient's chosen form before the service with the disclaimers (an estimate, not a guarantee; the final amount depends on the services rendered and the claim), and the patient is offered to pay the expected share or part of it, or to enrol in a payment plan, through the deposit and plan processes.

  4. Write back

The estimate with its version, its assumptions and its delivery record is written to the billing system on the account, the pre-service collection refers to it, and the final bill is compared to it so a large variance is explained to the patient rather than discovered; a flagged estimate goes to financial counseling in the Decision Desk.

Where it ends: The insured patient received their expected share before the service, computed from their benefits and the contract with the assumptions stated, and the billing system holds the estimate that the collection and the final bill refer to. The Decision Trace shows the benefits, the rate and the computation; the Audit Trace shows the eligibility call, the delivery and the write with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

The booking in any channel or the patient's request; the patient receives, pays or picks a plan on their phone.

The back office

Financial counseling sees estimates and exceptions; billing compares from the record.

The Decision Desk

  • A service without a contracted rate, a benefit the payer could not return, or an out-of-network provider on the case goes to financial counseling in the Decision Desk with the estimate and only the allowed outcomes.

Questions

What buyers ask next,answered plainly.

Where does the figure come from?

From the patient's benefits (deductible met, coinsurance, copay, out-of-pocket remaining) retrieved through the eligibility transaction, applied to the payer's contracted rate for the service and setting.

Is it a guarantee?

No. The estimate states its assumptions and the disclaimers; the final amount depends on the services rendered and the claim, and a large variance is explained against the estimate on file.

Can the patient pay from it?

Yes. The estimate offers payment of the expected share or part of it, or a payment plan, through the deposit and plan processes.
