Pre-op instructions and acknowledgment

How do we deliver the fasting, medication and arrival instructions before a procedure, get the patient's acknowledgment, and keep it in the record?

Generate the instructions from the case and make the acknowledgment a record. Before a procedure, Callvu builds the instructions from the EHR and the anesthesia plan (the fasting times by the fasting guideline and the procedure time, the medications to hold or take with the dates from the anesthesia assessment and the clearance, the arrival time and location, the escort and the ride home requirement, the skin preparation, what to bring and what to leave), delivers them to the patient's phone in their language at the department's intervals, chases the acknowledgment of each instruction with reminders, captures the patient's questions and routes them to the pre-operative nurse in the Decision Desk, and writes the instructions as sent and each acknowledgment with its time to the EHR so the day-of-surgery checklist reads them.

The problem today

The patient ate breakfast because the instruction sheet said midnight and the surgery had been moved to the afternoon.

Today, without Callvu

Pre-operative instructions are a printed sheet handed over at the consult, weeks before, with a fasting time for a case time that later changes, a medication list the patient reads differently from the nurse, and no record that the patient read any of it. The nurse's call the day before reaches a voicemail. A case is cancelled for a full stomach or a blood thinner, or the patient arrives without a ride and cannot be discharged.

The instructions have sources: the fasting guideline (the hours for clear liquids, light meals and solids before the anesthesia time), the medication plan from the anesthesia assessment and the clearance (what to stop and when, what to take with a sip), the arrival time derived from the case time, the escort rule for sedation, the skin preparation the surgical site infection protocol requires. The universal protocol and the accreditation standards expect the patient's understanding to be confirmed. The record should show what was sent, when, and that the patient acknowledged it.

Instructions generated from the case and acknowledged on the phone follow the case when it moves.

How it runs

From the requestto the system of record.

A procedure is booked and the anesthesia plan and the clearance are in the EHR, or the case time changes. Callvu builds or rebuilds the instructions.

Patient or caregiver
Reads and acknowledges each instruction on their phone and asks questions.
Pre-operative nurse
Answers questions and decides exceptions in the Decision Desk.
Callvu
Builds the instructions from the case, delivers, chases acknowledgments, routes questions and writes to the EHR.
  1. Build the instructions from the case

The case time, the procedure, the anesthesia type, the medication plan from the anesthesia assessment and the clearance, the surgical site preparation protocol and the facility's arrival rules generate the instructions: the fasting times computed from the fasting guideline and the anesthesia time (clear liquids, light meal, solids), the medications to hold with dates and the ones to take with a sip, the arrival time and location with parking, the escort and ride home requirement for sedation or general anesthesia, the skin preparation steps, what to bring and what to leave at home; a change to the case time rebuilds and resends the time-dependent items.

  2. Deliver and chase the acknowledgment

The instructions go to the patient's or the caregiver's phone in their language at the department's intervals (at booking, a week before, the day before), each instruction acknowledged with a tap; reminders chase the unacknowledged items; the patient can ask a question on each item, and an answer from the nurse is recorded with it.

  3. Chase and verify understanding

The day before, the time-critical items (the fasting start, the medications held, the escort confirmed with a name and a number) are confirmed with a short check; a patient who has not acknowledged by the cutoff, who reports taking a held medication, who has no escort, or whose question needs clinical judgment goes to the pre-operative nurse in the Decision Desk with the item and only the allowed outcomes (call the patient, amend the instruction with anesthesia, notify the surgeon, or flag the checklist).

  4. Write back

The instructions as sent with their versions and times, each acknowledgment with its time and device, the questions and answers, and the day-before confirmations are written to the EHR against the case, and the day-of-surgery checklist reads the acknowledgment status.

Where it ends: The patient received instructions generated from their case, acknowledged each one on their phone, confirmed the time-critical items the day before, and the EHR holds the record the checklist and an audit can read. The Decision Trace shows the instruction sources and any nurse decision; the Audit Trace shows each delivery, acknowledgment and confirmation with time.

Runtime enforcement

What is enforcedwhile the work runs.

The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.

The handoff

Between the front doorand the back office.

The front door

Texts or portal messages to the patient's or caregiver's phone at the department's intervals, in their language.

The back office

The pre-operative nurse answers questions and handles exceptions; the checklist reads the acknowledgments.

The Decision Desk

  • An unacknowledged item at the cutoff, a held medication taken, a missing escort or a clinical question goes to the pre-operative nurse in the Decision Desk with the item and only call the patient, amend with anesthesia, notify the surgeon, or flag the checklist.

Questions

What buyers ask next,answered plainly.

How are the fasting times set?

Computed from the anesthesia time under the fasting guideline (clear liquids, light meal, solids), and recomputed and resent if the case time changes.

How is the acknowledgment recorded?

Each instruction is acknowledged with a tap on the patient's phone, with the time and the device, and the time-critical items are confirmed again the day before; all of it is in the EHR against the case.

What if the patient took a medication they were told to hold?

The day-before check captures it and routes it to the pre-operative nurse, who decides with anesthesia before the morning.
