HIPAA authorizations
How do we capture a patient's HIPAA authorization to disclose their information with the recipients, the scope and the expiry defined and signed, and enforce it when records are released?
Capture the authorization as structured data the release process can read. When a patient authorizes a disclosure, Callvu prefills the patient and the record from the EHR, takes the authorization's required elements on a secure screen (the recipient by name or class, the information to be disclosed by type and date range, the purpose, the expiration date or event, the right to revoke and how, the redisclosure notice, the conditioning statement), verifies the signer's identity and authority (the patient or the personal representative), captures the signature and the date, chases an incomplete authorization for the missing element rather than filing a defective one, routes a sensitive category (substance use disorder records, psychotherapy notes, HIV status, genetic information) to the privacy office in the Decision Desk, stores the authorization as structured data in the EHR, and enforces its scope and expiry on every release. The problem today
The authorization on file named 'my lawyer', expired last year, and the records went out anyway.
A HIPAA authorization is a legal instrument with required elements, and most are captured as a paper form with blanks: a recipient described vaguely, a scope of 'all records', no expiration, a signature without a verified signer. The release of information desk reads the form by eye under time pressure and releases, or refuses, inconsistently. An invalid authorization makes the disclosure a violation; an expired one too.
The rule lists the core elements and the required statements, the signer's authority, the defective authorizations that cannot be relied on, the sensitive categories with their own rules (psychotherapy notes, substance use disorder records under their own regulation, state-protected categories), and the patient's right to revoke. Captured as structured data, the authorization can be checked for validity at signing and enforced automatically at each release: the recipient, the scope, the date range and the expiry.
An authorization the release process can read is one the hospital can rely on.
How it runs
From the requestto the system of record.
A patient (or their personal representative) wants to authorize a disclosure, from the portal, a call, a clinic or a release request. Callvu opens the authorization with the patient's record.
- Patient or personal representative
- Defines the recipient, the scope, the purpose and the expiry, and signs on a secure screen with identity verified.
- Privacy office
- Decides sensitive categories and authority questions in the Decision Desk.
- Release of information
- Releases under the stored authorization's scope through its process.
- Callvu
- Prefills, takes the elements, validates, verifies the signer, captures, chases, routes, stores and enforces.
Prefill and take the elements
The patient and the record are prefilled from the EHR; the authorization's elements are taken on a secure screen in plain terms: the recipient (a named person or organization with contact details, or a class), the information to be disclosed (by record type and date range, with the sensitive categories as separate explicit choices), the purpose (or 'at the request of the individual'), the expiration date or event, the right to revoke and how, the redisclosure notice, whether treatment or payment is conditioned on it (and that it is not, where it is not), and the signer's relationship.
Chase and verify
The signer's identity is verified and their authority classified (the patient, a parent for a minor subject to the state's minor consent exceptions, a personal representative with the document); the authorization is validated for the core elements and the required statements before it can be signed; a missing element is chased from the signer with the specific gap rather than filed defective; the signature and the date are captured.
Decide
A standard authorization proceeds by rule; one that includes psychotherapy notes, substance use disorder records (which need the separate consent under their own regulation), HIV or genetic information under state law, a research purpose, or a signer whose authority is unclear goes to the privacy office in the Decision Desk with the authorization and only the allowed outcomes (accept, require the specific consent, limit the scope, or reject with the reason to the patient).
Write back and enforce
The authorization is stored in the EHR as structured data (the recipient, the scope, the date range, the purpose, the expiry, the signer) linked to the record, and every release of information request is checked against it: the recipient matches, the records requested fall within the scope and the date range, the authorization has not expired or been revoked, and the sensitive categories are included only where explicitly authorized; a revocation is captured the same way and stops future releases.
Where it ends: The authorization was captured with every required element, signed by a verified, authorized person, stored as data, and enforced on each release with the scope and the expiry checked automatically. The Decision Trace shows the elements, the validation, the authority and any decision; the Audit Trace shows the signature, the storage and each release checked against it.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
The portal, a call with a link, a clinic tablet or a release request; the patient or representative completes on a secure screen.
The back office
The privacy office decides only sensitive categories and authority questions; release of information relies on the stored data.
The Decision Desk
- An authorization that includes psychotherapy notes, substance use disorder records, state-protected categories, a research purpose or an unclear signer goes to the privacy office in the Decision Desk with the authorization and only accept, require the specific consent, limit the scope, or reject with the reason.

Questions
What buyers ask next,answered plainly.
What must the authorization contain?
The core elements and statements the rule lists: the information described specifically, who may disclose and who may receive it, the purpose, an expiration, the signature and date, the right to revoke, the conditioning statement and the redisclosure notice; the process validates them before signing.
How is it enforced?
It is stored as structured data in the EHR, and every release request is checked against the recipient, the scope, the date range, the expiry and any revocation before records go out.
What about substance use disorder or mental health records?
They are explicit choices that route to the privacy office, and substance use disorder records from a Part 2 program need the Part 2 consent with its own elements.
