Billing disputes
How do we capture a patient's billing dispute from a call or the portal, classify it, route it to billing, and hold the account while it is open with the resolution recorded?
Make the dispute a case that holds the account. When a patient disputes a bill, Callvu identifies them, prefills the accounts and the disputed statement, captures the charges disputed and the reason in the patient's words, classifies the dispute (a coding or charge error, a coverage or claim issue, a surprise bill under the No Surprises Act, an estimate variance, a duplicate, a financial assistance question), places the hold on the account so statements and collection stop, routes the case to the billing specialist in the Decision Desk with the account history and the records, chases what is needed from coding, the payer or the patient, and writes the resolution with the adjustment or the explanation to the billing system with the notice to the patient and the hold released.

The problem today
The patient disputed the bill in March, the account went to the agency in April, and the adjustment was approved in May.
A billing dispute is a request to be heard and a signal that something may be wrong. In most hospitals it is a note on a call, an email to billing and a promise to call back; the account keeps aging; the collection letters keep coming; the dispute is resolved in the patient's favor after the damage. Under the surprise billing and debt collection rules, continuing to collect a disputed bill is also a legal exposure.
Disputes have types with different owners and rules: a coding error goes to coding, a claim issue to the payer, a surprise bill to the No Surprises Act process with its protections, an estimate variance to the dispute resolution rules, a financial assistance question to that process. Each needs the account held while open, a decision with reasons, and a written resolution. The case is the record the regulator and the patient will both ask for.
A dispute that holds the account the moment it is raised is the only kind that does no harm while it is decided.
How it runs
From the requestto the system of record.
A patient disputes a charge on a call, in the portal, by letter or through an AI assistant. Callvu identifies them and opens the dispute.
- Patient or guarantor
- Describes the disputed charges and the reason, provides documents through a link, and receives the resolution.
- Billing specialist
- Investigates and decides in the Decision Desk.
- Coding, the payer and clinical departments
- Answer the questions the dispute raises through their links.
- Callvu
- Prefills, captures, classifies, holds, routes, chases and writes.
Prefill and capture
The accounts, the statements, the claims and the payments, any estimate on file and the insurance are prefilled from the billing system; the patient marks the charges disputed and states the reason in their own words, uploads any document (an explanation of benefits, an estimate, a receipt) through a link.
Classify and hold
The dispute is classified: a coding or charge error, a claim or coverage issue, a surprise bill (an out-of-network provider at an in-network facility or an emergency), a self-pay bill above the good faith estimate, a duplicate or a posting error, or a financial assistance question; the account is placed on hold so statements, collection calls and agency placement stop while the dispute is open, and the acknowledgment goes to the patient with the case number.
Chase and decide
The case goes to the billing specialist in the Decision Desk with the account history, the claim and the records; what the specialist needs is chased from coding (a chart review), the payer (the adjudication detail), the clinical department (the service rendered) or the patient; the specialist decides with only the allowed outcomes: adjust with the reason, rebill, refer to the surprise billing or estimate dispute process, refer to financial assistance, or uphold with the explanation.
Write back
The resolution is written to the billing system with the adjustment, the rebill or the explanation, the written notice goes to the patient with the reasons and any further rights, the hold is released or converted to a plan, and the dispute is logged by type for the compliance report and the root cause review.
Where it ends: The dispute was captured in the patient's words, classified to its owner, held the account while open, decided with reasons, and the billing system holds the resolution with the notice sent. The Decision Trace shows the classification and the decision; the Audit Trace shows the capture, the hold, the chases and the write with time.
Runtime enforcement
What is enforcedwhile the work runs.
The regulations preloaded in Callvu Studio and the bank's own policies, applied as steps that run, not lines in a binder. A step that breaks a rule does not run, so a completed run was compliant.
The handoff
Between the front doorand the back office.
The front door
A call, the portal, a letter or an AI assistant; the patient describes and uploads on their phone.
The back office
The billing specialist decides with the history and the answers from coding and the payer; collection reads the hold.
The Decision Desk
- Every dispute goes to the billing specialist in the Decision Desk with the account history and the records, and only adjust with reason, rebill, refer to the surprise billing or estimate process, refer to financial assistance, or uphold with the explanation.

Questions
What buyers ask next,answered plainly.
Does collection stop while the dispute is open?
Yes. The account is held from the moment the dispute is raised, so statements, calls and agency placement stop until the resolution, as the debt collection and surprise billing rules expect.
Who decides?
The billing specialist in the Decision Desk with the account history, the claim and the answers chased from coding, the payer or the department, with only the allowed outcomes.
What does the patient receive?
An acknowledgment with the case number, and a written resolution with the reasons and any further rights, such as the surprise billing or estimate dispute processes.
